  • You are asked to contact the Practice to request registration. You will be asked where you live to ensure that you are within our geographic boundary.
  • Patient detail and application forms will be sent to you or may be collected. These should be completed and returned to us, as soon as possible, to enable us to register you.
  • We do not discriminate on grounds of gender, marital status, race, ethnic origin, colour, nationality, national origin, disability, sexual orientation, religion or age. We oppose all forms of unlawful and unfair discrimination.
  • We do not operate a fixed patient list system. This means that all of our patients are free to book their appointments with whichever of our Doctors they wish, subject to that particular Doctors availability at any given time.

Patients living within the practice area may register with Lincoln House Surgery.

When you register you will be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online copy of this file, which you may print off, fill out and return to us as soon as possible.
