We would like to know how we can improve our service to you and we would welcome your thoughts about our surgery & staff.

To help us to achieve this, we have formed patient participation group so that you can have your say. We will ask members of this group some questions from time to time, such as what you think about our opening times or the quality of the care or service you receive.

We aim to gather a number of patients from as broad a spectrum as possible to get a truly representative sample of responses. We need everyone - young people, workers, retirees, people suffering from long-term conditions & people from non-British ethnic groups.

If you would like to join our patient participation group and have your say, please ask for a form at reception or complete the attached form and leave your details.

We sent out a questionnaire asking for our patient's help in planning our next annual survey. In order to ensure that we asked the right questions, we asked you how we could improve our services in the following areas:

We have managed to gather all the details from the questionnaire on key areas to focus in our First Annual Survey at Lincoln House Surgery.

As a result of this we have had our first Patient Satisfaction Questionnaire which he had handed out in the form of paper forms as well as an online Questionnaire. A copy of the Questionnaire is attached below.

Our First Annual Survey is finally complete and the results have been published. We received a total of 208 completed forms including online & handout questionnaires.

We would now like to invite members of our Patient Participation Group to an online forum for discussions, with the aim of successfully determining what actions we can take to improve the services that we offer you. All the PPG members have been invited via the email addresses registered.

For those of you who would rather prefer a face-to-face meeting of the Patient Participation Group, we would soon be announcing a date.
